Job Summary We are seeking an autonomous, compassionate Nurse Practitioner to deliver comprehensive primary and preventative care across diverse clinical, facility, and home settings. Collaborating with multidisciplinary teams, you will manage complex medical needs, administer treatments, and support patient recovery. Key Responsibilities
- Comprehensive Care: Conduct thorough patient assessments, perform physical examinations, and develop individualized care plans.
- Diagnosis & Procedures: Order, perform, and interpret diagnostic tests. Assist with and perform procedures such as phlebotomy, laboratory testing, and pain management techniques.
- Prescriptive Authority & Medication: Prescribe and administer medications accurately, strictly following medical orders, provincial regulations, and college standards.
- Patient Monitoring: Monitor patient vital signs and respond promptly to any changes in clinical condition.
- Specialized Care Management: Assist in managing chronic conditions, pediatric health issues, and senior care needs. Support patients with dementia or other cognitive impairments by implementing specialized care strategies.
- Collaboration & Case Management: Coordinate seamlessly with physicians, case managers, and other healthcare professionals to optimize patient outcomes.
- Patient Education: Counsel patients and families regarding health conditions, medication use, post-discharge care, and healthy lifestyle choices.
- Documentation & Safety: Document patient information accurately using electronic health record (EHR) systems such as Epic or Cerner. Maintain a safe, clean environment while strictly adhering to infection control standards.
Qualifications & Skills
- Education: Master’s Degree in Nursing (Nurse Practitioner stream) from an accredited program.
- Licensure: Current, unencumbered registration as a Nurse Practitioner in the Extended Class with the provincial College of Nurses.
- Clinical Experience: Proven experience working with children, seniors, or in acute care environments such as hospitals or nursing homes. Familiarity with medical office procedures and home care services is a strong plus.
- Technical Proficiency: Strong knowledge of medical terminology and medication administration protocols. Experience with EHR systems like Epic or Cerner is highly preferred.
- Specialized Skills: Skilled in case management, pain management, phlebotomy, laboratory testing, and dementia/behavioral health care.
- Attributes: Excellent patient care skills with compassionate communication abilities. Strong organizational skills, sharp attention to detail, and the ability to work effectively within multidisciplinary teams in fast-paced settings.
