The LPN provides competent, holistic care to clients of all ages attending treatment at Thorpe. This position supports client’s recovery by maintaining the medical health of each client through the assessment, consultation, and administration of controlled, prescribed, and OTC medications.
Responsibilities and Duties
Medication:

  • Dispenses medication at scheduled times throughout the day and reconciles any discrepancies.
  • Obtains medication history on admission and reconciles medications.
  • Transfers prescriptions between pharmacies and arranges delivery as required.
  • Ensures controlled medications are counted, stored and ordered from pharmacy prn.
  • Follows standing orders as per facility protocol.
  • Contacts/consults and assists TRC’s Nurse Practitioner as required.
  • Reports and tracks medication issues.
  • Medication ordering.
  • Finding coverage for clients who otherwise have no means of payment.
  • Ensuring client comes with all RX’s needed; if not it is the LPN’s job to navigate how to obtain these RX’s.

Admissions/Transfers/Discharges

  • Obtains and reviews all medical history forms during admissions, and prioritizes health issues.
  • Stores all client medications appropriately.
  • Provides proper documentation for medication when patients transfer to another facility.
  • On discharge, reconciles medications and personal belongings to clients.
  • Completes drug screens during treatment.
  • Orders ward stock required for the Nursing department.

C*lient Care*

  • Monitors vital signs.
  • Assist the General Nurse Practitioner with zoom calls.
  • Provides information and support to clients that are relevant including: medications, STI, pregnancy, communicable diseases, etc.
  • Provides education to clients regarding health concerns and other medical related issues.
  • Books doctor appointments when appropriate for clients; transfers paperwork to the appropriate doctor, and ensures follow-up appointments have been
  • Coordinates with other clinics for test results to be sent to TRC.
  • Communicates medical information to counsellors by email or in person.
  • Provides referrals as necessary.
  • Works closely with Nurse Manager regarding concerns addressed by the client.
  • Documents client case notes, medications, special requirements and care for the client.
  • Attends shift change meetings and reports on client progress.

Other Duties

  • Ensures that the cupboard is stocked with medication cups, first aid supplies as well as over-the-counter medications.
  • Conducts pre-employment drug testing for potential staff members.
  • Provides active listening and validation to clients.
  • Regularly checks emergency equipment including the Automated External Defibrillator (AED) device and pads.
  • All other related duties as required by the Nurse Manager or Clinical Director.

Qualifications, Experience and Skills

  • Practical Nurse Diploma from a recognized post-secondary practical nurse program
  • Maintain a permanent, valid practice permit with the College of Licensed Practical Nurses of Alberta in good standing.
  • First Aid up-to-date.
  • Must be willing to work full scope of practice (including IM/SC/ IV)
  • Suboxone Course- to be completed ASAP after employment commences.
  • Experience in an addiction centre preferred
  • Team player/able to work independently when needed
  • Able to work 12 hour shifts to include weekends and nights.
