Altitude Healthcare is a staffing agency that supports Long Term Care (LTC) homes and hospitals who are experiencing staffing shortages. Our staffing strategy is built for long-term growth and sustainability. We are currently looking for Registered Nurse to join our dedicated, caring and energetic team of support staff. Long-term assignments and travel opportunities are available.
Responsibilities:
- Assists with the provision of nursing care in accordance with established standards of nursing practice set by the College of Nurses of Ontario.
- Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities according to the CNO standards of practice for NP’s
- Facilitates continuity of care by working collaboratively with professional health care teams including primary care providers
- Contributes to the assessment and documentation of each resident‘s health status on an ongoing basis.
- Communicates finding and diagnoses, discusses health outcomes to resident, families and the care team
- Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness. Communicates effectively with residents, staff, families, physicians and team members.
- Provides excellent patient care at all times along with individualized service.
- Observes reports and records symptoms and conditions of residents.
- Encourages and models critical thinking that identifies needs and makes recommendations for changes within the home to enhance best practice outcomes
- Consults with a physician in accordance with the standards for consultation with physicians
- Participates in Quality Improvement and Risk Management activities.
- Performs other duties as assigned.
Qualifications:
- Valid registration as RN (EC)- entitled to practice with no restrictions
- Nurse Practitioner Certification from recognized program
- Demonstrated competency in advanced assessments, diagnoses and prescribing pharmaceutical and non non-pharmacological interventions within the legislated scope of practice.
- A valid police check for the vulnerable sector- must be dated within six (6) months.
- TB test within the last 12 months
- Covid -19 Immunization Documentation
- Immunization Record (TdaP, MMR, Varicella/Zoster, TST/Mantoux, Hepatitis B)
- Excellent communication and organizational skills
- Must be legally authorized to work in Canada
