Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care, conducting assessments, and developing treatment plans. This role requires a strong background in medical office operations, case management, and experience with diverse patient populations, including children and seniors.
Duties
- Conduct comprehensive physical examinations and assessments of patients.
- Develop and implement individualized care plans based on patient needs.
- Perform diagnostic tests, including phlebotomy and laboratory procedures.
- Provide education to patients and families regarding health conditions and treatment options.
- Manage chronic diseases and coordinate care for patients with complex medical needs.
- Document all patient interactions accurately in the electronic health record system (Epic).
- Collaborate with other healthcare professionals to ensure optimal patient outcomes.
- Provide specialized care for patients with dementia and other age-related conditions.
- Maintain a safe and compliant medical office environment.
Qualifications
- Valid Nurse Practitioner license in the state of practice.
- Experience in a medical office setting is preferred.
- Strong skills in patient care, laboratory procedures, and case management.
- Experience working with children and seniors is highly desirable.
- Proficiency in using electronic health record systems, particularly Epic.
- Excellent communication skills and ability to work collaboratively within a team.

Job Types: Part-time, Fixed term contract
Contract length: 18 months
Pay: $65.00-$75.00 per hour
