The Mount Sinai Academic Family Health Team – Toronto site is looking for a Nurse Practitioner to join the team to support us on our journey. In collaboration with the interdisciplinary team, the Nurse Practitioner – Primary Care will be responsible for providing primary care to patients in the Mount Sinai Academic Family Health Team . The Nurse Practitioner will perform clinical duties within their scope of practice, including assessments, diagnosis, treatment, follow up, counseling, screening, education and referral. These duties will be performed at the clinic through scheduled virtual and in-person appointments for all patients at this site of our Academic Family Health Team.
This role will also be a key member of the Mount Sinai Academic Health Team’s Healthy Living with Chronic Pain (HeLP) program. This program aims to address the challenge of providing high quality patient care for those living with chronic non-cancer pain on opioid medication(s) in resident practices. The NP in this program will specialize in complex pain management using a holistic approach to assessing pain for each individual and taking into account not only specific disease processes but also the biological, psychological, spiritual, and sociological factors for each patient.
