The Registered Nurse (RN) is an essential member of the Rapid Access Addiction Medicine (RAAM) interdisciplinary, recovery oriented care team. RAAM provides low barrier, evidence based addiction medicine services to individuals experiencing substance use and concurrent mental health challenges. The RN supports rapid access to care through same day screening, comprehensive nursing assessments, care coordination, and clinical interventions. Working collaboratively with physicians, addiction counsellors, social workers, primary care paramedics, and peer support workers, the RN delivers trauma informed, culturally safe, and patient centered care across the addiction treatment continuum. Key responsibilities include completing nursing health assessments; supporting intake, treatment planning, referrals, and follow up; implementing nursing and medication interventions within scope; assisting physicians with virtual and in person care; providing education and advocacy; coordinating transitions of care; and maintaining accurate clinical documentation. The RN contributes to quality improvement, safety initiatives, and interdisciplinary decision making while supporting clients’ recovery journeys.
