Nurse Practitioner
Full-Time Contract (Maternity Leave)
The Southlake Academic Family Health Team (FHT) is seeking a dynamic Primary Care Nurse Practitioner to join our multidisciplinary primary care team. The incumbents will provide comprehensive primary care services within the RN (EC) scope of practice. These services will include core primary care, health promotion, disease prevention and chronic disease management as supported by the larger inter-disciplinary care team. The position will be 37.5 hours weekly and will be based primarily out of our Newmarket and/or Mount Albert clinics. This position is available starting December 2024.
Duties:
- Individual in-person and virtual patient assessment and treatment, including screening and preventative activities for both planned and episodic care visits (including urgent care).
- Assisting the team with program development to meet the needs of our patient population
- Promoting quality improvement initiatives which enhance care delivery processes and patient outcomes within the FHT
- Supporting inter-disciplinary learners as they rotate through the clinic
- Home visits
- Other duties within the scope of a Nurse Practitioner
Qualifications:
- Current RN-EC Certificate of Competence (College of Nursesof Ontario)
- Preference will be given to candidates with primary care experience
- Demonstrated ability to work in a team environment
