Job Overview
Nuvella Haven Care is seeking an experienced Complex Needs Coordinator to support adults with developmental disabilities and complex service needs as they transition from hospitals to community-based homes.
This role serves as the primary coordinator responsible for discharge planning, stabilization, risk management, behavioural support coordination, and long-term community integration.
Many individuals supported through this program may present with:
- Aggression
- Self-injurious behaviours
- Property destruction
- Mental health concerns
- Addiction histories
- Trauma backgrounds
- Forensic involvement
- Complex medical needs
The successful candidate must be confident managing high-risk situations while coordinating multidisciplinary supports and promoting positive outcomes.
KEY RESPONSIBILITIES
Transition Planning
- Participate in hospital discharge meetings
- Coordinate safe community transitions
- Develop individualized transition plans
- Support move-in preparation and stabilization
Case Coordination
- Coordinate services across multiple providers
- Facilitate case conferences
- Maintain communication with families and guardians
- Monitor service effectiveness
Behavioural & Crisis Support
- Support implementation of behaviour plans
- Coordinate crisis prevention strategies
- Monitor behavioural trends and risk factors
- Participate in incident reviews
Community Integration
- Support community inclusion and engagement
- Promote independence and quality of life
- Assist clients in accessing community resources
Documentation & Reporting
- Maintain accurate records
- Complete progress reports
- Participate in quality assurance reviews
- Track outcomes and stabilization measures
QUALIFICATIONS
Required:
- Degree or diploma in Social Work, Human Services, Psychology, Disability Studies, Community Rehabilitation, Nursing, or related field
- Minimum 5 years experience supporting individuals with developmental disabilities
- Minimum 3 years experience coordinating complex cases
- Experience supporting behavioural and mental health needs
- Strong crisis management skills
- Valid Class 5 driver's licence
Preferred:
- Hospital discharge experience
- PDD experience
- Mental health and addiction experience
- Behaviour support experience
- Experience working with multidisciplinary teams
Pay: $75,000.00-$82,000.00 per year
