Position: Social Worker Bachelors

Status: Permanent Full-Time

Program: Critical and Emergency Care

Department: Medical Surgical ICU

Site: Ramsey Lake Health Centre (RLHC)

Purpose:
To advocate for and work with patients and families whose ability to function safely in
the community has been seriously compromised by illness or injury and mental health
challenges. To play a critical role in coordinating and facilitating a patients viable
discharge from hospital, through the identification of personal resources and
community services to optimize their function and reduce the risk of psychosocial
breakdown, and to avoid precarious discharges and unnecessary readmission to
hospital.

Education and

Training:
 Successful completion of a university Bachelor’s degree in Social Work
 Currently registered with the Ontario College of Social Workers and Social Service

Workers
Experience:  Recent and relevant experience (minimum 5 years) inpatient hospital experience
 Experience working with critical ill patients and families

Knowledge:  Demonstrated excelled interpersonal and communication skills with the proven
ability to communicate effectively with patients, families, physicians and other
health care professionals

 Proven ability to foster and improve relationships between professionals,
programs and agencies

 Provision of crisis intervention, emotional support and/or education to
patient/family experiencing new diagnosis, trauma, bereavement, family discord,
lifestyle changes and coping strategies

 Demonstrated knowledge of Office of the Public Guardian Trustee
 Demonstrated knowledge of community resources
 Knowledge and understanding of simple and complex (across country) repatriation

processes
 Proven problem solving skills

threatening medical conditions

 Proven ability to work well in a team setting
 Ability to respond well to changing environment
 Demonstrated positive work record and attendance record

Selection Process:
Candidates will be selected for this position on the basis of their skill, ability,
experience and relevant qualifications as identified in the resume and completed
Application Form submitted. Where these factors are relatively equal, seniority
shall govern. HSN reserves the right to conduct a formal interview/testing
process where required, which may include (but not limited to), a written or
practical examination or both to determine skill and ability.

Salary: $33.87$43.15/Hourly
