As part of a multi-disciplinary team on Internal Medicine, receiving functional supervision from the RN or Health Discipline, the clinical assistant will be responsible for contributing to patient care by providing basic patient support and for assisting with indirect care tasks.
RESPONSIBILITIES:
- Contributing to patient profile upon admission by supporting the patient upon entry
- liaise with RN at start of shift in assigned zone to receive report and coordinate care plans together
- receive and provide Transfer of Accountability (TOA) report with CA colleagues
- report any changes in patient's condition to RN (skin integrity, bowel status, acute changes, restlessness/agitation, etc.)
- Assisting patients with activities of daily living including proper hygiene care (bathing/showering), feeding/set-up for meals, toileting
- Providing proactive observation and frequent checks to encourage and engage patients in his/her care as required
- Providing support to the emergency team in response and restraint of the aggressive patient utilizing Crises Prevention Institute techniques
- taking patient weight and measure intake and output and report to RN
- Answering call lights/bells
- arranging for the proper safekeeping of patients' belongings
- Demonstrating the ability to show initiative, independence, priority setting and problem solving techniques in a time limited and chaotic environment, while maintaining composure
- transport patients and mobilizing patients as directed by RN/HD
- Assisting with cardiac arrest and trauma response
- assists with admissions, transfers and discharges as directed by RN/HD
- undertaking preparation of the deceased patient for the morgue
- Participating, with team members, in the patient care planning process and in the development and implementation of quality-based initiatives
- Transporting critically ill patients
- Being proactive and assisting team in identifying and mitigating risks
- set up rooms for admissions, stock linen cupboards throughout shift as time allows, stock isolation rooms/carts with supplies
