As an Unregulated Care Provider (UCP) at V!VA Leaside, you will play a meaningful role in supporting the health, wellness, comfort, and independence of our Community Members. Through your compassion, professionalism, and commitment to exceptional care, you will help create an environment where Community Members feel respected, valued, supported, and empowered to live life to the fullest.
Reporting to the Director of Care, Wellness Coordinator, Wellness Supervisor, or Wellness Lead, you will provide personal support and assistance with activities of daily living, monitor and report changes in condition, maintain accurate documentation, and collaborate with fellow Team Members to ensure exceptional care and service are delivered each day.
All care services are delivered in accordance with individual Care Plans, V!VA policies, and applicable legislative and regulatory requirements.
Observe and promptly report changes in Community Member health, well-being, behaviours, or care needs to the appropriate Wellness Team Member.
- Assist Community Members with activities of daily living, including personal hygiene, grooming, dressing, bathing, toileting, continence care, mobility support, transfers, dining assistance, companionship, and recreation activities.
- Support Community Members in maintaining their independence while providing assistance aligned with their individual needs and Care Plans.
Provide emotional support, companionship, and meaningful engagement opportunities that enhance the well-being and quality of life of Community Members.
- Accurately observe, record, and communicate information regarding Community Member care and well-being.
- Assist with Community Member move-ins, transfers, and transitions as required.
- Promote dignity, privacy, choice, and independence in every interaction.
- Maintain accurate, timely, and complete documentation in accordance with organizational policies and procedures.
- Ensure all required communication, documentation, and reporting requirements are completed accurately and promptly.
- Complete incident reports promptly and accurately when required.
- Maintain Community Member confidentiality, dignity, privacy, and professional boundaries at all times.
- Adhere to the Retirement Homes Act, RHRA requirements, and all V!VA policies, procedures, and wellness protocols.
- Respond appropriately to emergencies and escalate concerns to a supervisor or Emergency Medical Services as required.
- Maintain knowledge of emergency codes, policies, and procedures and participate in emergency drills and training.
- Follow infection prevention and control protocols, including outbreak management procedures when required.
- Promote a safe, clean, and comfortable environment for Community Members, visitors, and Team Members.
- Practice safe work habits and promptly report hazards, incidents, or safety concerns.
- Assist in maintaining a safe and secure environment for Community Members at all times.
- Collaborate with Wellness Team Members and Team Members across all departments to support exceptional service and quality care.
- Participate in departmental meetings, Town Hall meetings, daily Making Today Great meetings, and quality improvement initiatives.
- Contribute to committees and programs that support Occupational Health & Safety, Infection Control, Falls Prevention, and Community Member well-being.
- Support departmental goals and continuous improvement initiatives through active participation and feedback.
- Demonstrate V!VA's Orange Dot Values in every interaction.
- Foster positive relationships with Community Members, families, visitors, and Team Members while contributing to a culture of respect, collaboration, and excellence.
- Complete all required education, training, projects, and additional responsibilities as assigned.
Qualifications:
Successful completion of, or current enrollment in, a recognized Personal Support Worker (PSW), Developmental Support Worker (DSW), Health Care Aide, Nursing Student, or equivalent healthcare program.
- Experience in retirement living, seniors' care, healthcare, or a related setting is considered an asset.
- Knowledge of clinical documentation practices is considered an asset.
- Knowledge of Microsoft Outlook and Word is considered an asset.
- Strong communication, customer service, and interpersonal skills.
- Ability to work independently while contributing to a collaborative team environment.
- Ability to prioritize responsibilities and adapt in a fast-paced environment.
- Commitment to delivering exceptional, person-centred care and service.
- Ability to maintain confidentiality and exercise sound professional judgement.
- Able to effectively communicate in verbal and written English.
- Valid CPR/First Aid Certification required.
- Smart Serve Certification is considered an asset.
- Valid Vulnerable Sector Screening (VSS) required.
- Ability to safely lift, carry, push, and move up to 50 lbs.
- Ability to work overnight shifts, weekends, and statutory holidays as required.
- Available for call-ins and emergency coverage when required.
