Your Role:
  • Assess and provide consultation, recommendations and interventions to clients in their home using evidence based best practice
  • Ensure coordinated delivery of care through ongoing communication with client care team regarding plans and schedules for interventions.
  • Problem solve in collaboration with client and care team to create opportunities for more independence
Qualifications:
  • Member in good standing of the College of Occupational Therapists of Ontario.
  • Minimum 2 years clinical experience (optimal)
  • Valid Driver’s License with access to a vehicle

Since 2004, Champlain Rehab Solutions’ dedicated team of Occupational Therapists, Physiotherapists and Social Workers has offered outstanding care to the most vulnerable Ottawa adults and seniors living with disabilities, mental health disorders or suffering from a chronic or terminal illness. Your Role:

Qualifications:
