What We’re Looking For:
We are seeking skilled and compassionate Licensed Practical Nurses (LPNs) to join our team. Key Responsibilities
- Conduct wellness nurse home visits, providing 30–40 minute appointments to support stable patients managing chronic diseases and maintaining their independence.
- Deliver exceptional nursing care in accordance with individualized care plans, best practices, regulatory guidelines, and patient safety standards.
- Act as the eyes and ears of the physician/GP by monitoring patient status, identifying concerns early, and supporting preventative and treatment-based care.
- Perform comprehensive nursing assessments, including cognitive, cardiovascular, respiratory, lower limb, skin integrity, and general health assessments.
- Monitor patient progress and accurately document findings, interventions, and changes in condition.
- Address Activities of Daily Living (ADLs), including social well-being, mental health, sleep, mobility, personal care needs, and overall wellness.
- Provide patient and family education to promote wellness, recovery, independence, proper foot hygiene, footwear, and self-care strategies.
- Coordinate care on behalf of the GP, including arranging diagnostics, labs, referrals, and communication with interdisciplinary team members.
- Provide nursing services such as wound care, vaccinations, personal care, foot care, and charting by exception.
- Provide professional nursing foot care, including:
- Nail trimming and filing
- Callus and corn reduction
- Assessment and care for thickened, fungal, or ingrown nails
- Identification of potential complications such as ulcers, infections, circulation issues, wounds, or changes in skin integrity
- Conduct foot and lower limb assessments and develop personalized care plans based on client needs.
- Adhere to infection control, sterilization, confidentiality, and documentation protocols at all times.
- Communicate effectively with clients, caregivers, families, physicians, and interdisciplinary healthcare team members.
- Travel to client homes, community settings, or residential care facilities to provide care.
- Maintain required foot care supplies, nursing equipment, and documentation tools.
- Practice safely and competently within the limits of your own knowledge, training, experience, and professional scope of practice.
Skills and Qualifications:
o Positive, can-do attitude with excellent relational and observational skills.
o Proficiency in checking vital signs and conducting comprehensive assessments.
o Strong time management and attention to detail.
o Pharmacological knowledge.
o Current nursing license (LPN) and CPR-C certification.
o Reliable vehicle and a valid driver’s license for travel to patient locations.
o Completion of an Advanced Nursing Foot care certification.
Compensation:
- Part-Time $45/hour
