Job Summary:
St Joseph’s Hospital Hamilton is a leading academic teaching hospital and the first such Canadian research hospital to embrace EPIC, an international leader in health information technology. Our Clinical Pharmacology division is looking for a qualified Physician Assistant (PA) with experience in Internal Medicine wards and conversant in evidence-based therapeutics to support and expand the inpatient consult service. The service currently focuses on adult patients with suspected adverse drug reactions, polypharmacy and deprescribing, expensive non-formulary medications, as well as supporting the optimization management of high priority medications within our randomized trials.

The successful applicant will work with the Clinical Pharmacologist leads and the research team to serve as a point of contact and resource for the referred patients and the health care professionals in their circle of care. The role is to ensure optimal patient outcomes through effective identification, prevention and resolution of drug-therapy problems in the transition period from hospital to home. The successful applicant can expect to significantly advance their history taking, physical exam, diagnostic text interpretation, and therapy prioritization skills amongst multi-morbidity patients.

Involvement in research requires some additional knowledge and understanding of the study protocols, research ethics, and Good Clinical Practice.

Qualifications:
Graduate of an accredited PA Education Program in Canada or the United States
Certified through the Physician Assistant Certification Council of Canada (PACCC) or National Commission on Certification of Physician Assistants (NCCPA). If new PA program graduate, will accept application if eligible for certification.
Active membership with the Canadian Association of Physician Assistants
Critical appraisal training required
Experience or background in pharmacy a major asset
A degree or courses completed in health research methods, or quality improvement is a major asset
2 years clinical experience preferred
Computer use including word processing, spreadsheets, presentation software required
Ability to use electronic health records, Word and Excel is required

Responsibilities:
Work up of history, physical, investigations and notes of patients.
Performing detailed discharge medication reconciliations including adherence checking and initial identification of medication discrepancies, non-recorded medications and aberrant use; identification and follow-up of drug therapy problems; developing, documenting and communicating medication management plans with the Clinical Pharmacologist; participating in shared decision-making exercises with patients at participating sites include hospitals in our region within and outside of Hamilton
Patient and caregiver interviews, including assessment of engagement, to inform medication plan, completing structured patient education and clinical documentation.
Planning, scheduling and carrying out follow-up visits for patients according to the clinical or study requirements using telemedicine facilities including EPIC telemedicine ‘virtual visits’
Working with the research team to recruit patients, extract data from charts and enter in REDCap case study forms. Participating in research evaluation and knowledge translation activities
Providing coordination of care activities such as communicating to hospital personnel, family physician, community pharmacist, medical specialists, and other interdisciplinary team members as required. to ensure smooth transition for medication-related care between hospital and post-discharge community care
Other duties as assigned

Hours of Work:
:2-3 days per week between Monday and Friday
