Navigator

We are seeking a motivated team player with a background in the community support services to join our Assisted Living team as a Navigator. Under the director of the Manager, Assisted Living Services, the Navigator assess client’s eligibility for service, determines appropriate supports and provides ongoing assessment to clients as their needs change and service requires increase or decrease.

The Navigator provides client service delivery by:

  • Monitoring waitlist and communicating with clients
  • Facilitating communication and collaboration amongst the client’s circle of care
  • Complete assessments via telephone or in-home where appropriate
  • Assessing, planning, coordinating, implementing and reviewing client needs and services
  • Identifying risk factors and determining urgency of care
  • Develop client centred care plans, enabling clients to remain home in their communities
  • Provide information to clients and offer referrals to Community Care Durham services and alternate community resources where needed
  • Processing client discharges
  • Participate in agency awareness initiatives and community events
  • Due to the nature of the role, responsibilities may be revised

Qualifications required:

  • Post-secondary degree in Gerontology or related field (e.g., Social Work, Psychology, Mental Health Services, Human Services)
  • Minimum two years’ recent experience in community health or a related field (acute, hospice, home and community care settings).
  • Registration with a regulatory body an asset (i.e. College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, College of Audiologists and Speech Language Pathologists of Ontario, Ontario College of Social Workers and Social Services Workers)
  • Knowledge of community resources and demonstrated ability to collaborate and establish and strengthen care teams
  • Knowledge and experience in Care Coordination, including assessment, care planning, system navigation, health outcomes monitoring, collaboration with key system partners
  • Knowledge of OCAN, RAI-CHA, IAR and Health Links Coordinated Care Plans an asset
  • Knowledge of CIMS Database an asset
  • Demonstrated skill and experience in providing care in a manner that is culturally responsive and sensitive to the needs of individuals from various backgrounds and diversity.
  • Intermediate computer skills required
  • Valid driver’s license and access to a reliable vehicle
  • Insurance that includes driving for business purposes and minimum liability of $1,000,000.
  • Ability to communicate in a second language is an asset

Job Type: Full-time

Salary: $19.73 /hour

  • Gerontology or related field: 1 year (Preferred)
