Description
Position Summary
Practices as a member of the multidisciplinary team, under the standards of The College of Nurses of Ontario, Accreditation Canada, the Ministry of Health, Ministry of Long-Term Care and policies of Deep River & District Health (DRDH).
Position Relationships
Reports directly to the Manager of Clinical Services of the area they are assigned to. Collaborates with staff, physicians, families and residents/patients, as well as external care providers to deliver care in response to the identified needs of patients and residents.
Duties:
- Practice as a member of the multidisciplinary team to provide quality clinical care and teaching to patients, residents, and their families.
- Adheres to all policies and procedures.
- Supports and demonstrates the mission, vision and values of the organization.
- Other duties may be assigned and may not be limited to the scope of an individual department.
Qualifications:
- Current Registration with the College of Nurses of Ontario as a Registered Practical Nurse, and be a member in good standing required.
- Practical Nursing Diploma or equivalent education and relevant experience.
- Current certification in Basic Life Support (BLS).
- 12 Lead ECG Interpretation Course or equivalent preferred.
- Gentle Persuasive Approach certificate preferred.
- Education and training related to dementia and geriatric care preferred: included but not limited to Montessori, restorative care approaches, palliative care, etc.
- Recent clinical experience in an acute, primary, or long term care setting.
- Experience leading interdisciplinary teams preferred.
- Experience supervising and directing the care of unregulated health care providers.
- Knowledge of LTC policies/legislation on assessment and care planning processes.
- Knowledge of current trends in nursing and health care required.
- Demonstrated exemplary documentation skills and holistic assessment skills required.
- Knowledge of applicable legislative and hospital/program policies required.
- Exceptional Attendance required.
- Must agree to adhere to the standards of behaviour of the organization.
