Job Summary
We are seeking a dedicated and knowledgeable relief Pharmacist for 2 weeks starting August 1, Monday to Saturday. The hours of the store are from 10 am to 6 pm on weekdays and 10 am to 2 pm on Saturday. The exact dates and length of employment are not yet finalized and are susceptible to change.
Duties
- Review and interpret medication orders to ensure accuracy and appropriateness for patient needs.
- Provide medication counseling to patients and healthcare providers, explaining proper usage, potential side effects, and interactions.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Monitor patient outcomes and adjust medication regimens as necessary to optimize therapeutic results.
- Maintain accurate records of medications dispensed, including inventory management and compliance with regulatory standards.
- Educate patients on health management strategies related to their medications and overall wellness.
- Participate in clinical rounds with medical staff to provide pharmaceutical expertise in patient care decisions.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Active pharmacist license in Ontario.
- Experience in acute care settings, particularly with pediatric patients, is preferred.
- Strong knowledge of medical terminology and medication administration practices.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to work collaboratively in a fast-paced hospital environment while maintaining attention to detail.
