Integrated Services for Seniors Case Worker
TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking one Case Worker to join our team in a full-time permanent role.
A Bachelor’s of Social Work (BSW) or similar is required for this role, and the ideal candidate will have 2-3 years of experience working with vulnerable clients. Registration with the Ontario College of Social Workers and Social Service Workers is also required. The ideal candidate will have two to three years of experience working with vulnerable clients, with experience working with seniors considered an asset. Strong interpersonal skills and the ability to build and maintain positive relationships with peers, partners, and diverse community stakeholders are essential. Experience working in fast-paced, client-centred environments is required. Proficiency with MS Office applications and standard office equipment, as well as the ability to design flyers and presentations is necessary. The successful candidate will possess strong oral and written communication skills in English. Proficiency in additional languages, particularly Mandarin, Bengali, or Tamil, is considered an asset. The candidate will demonstrate a commitment to maintaining the confidentiality and security of client records and be willing to travel throughout the city as required.
DUTIES & RESPONSIBILITIES
- Conduct individual needs assessment and develop a case management plan for each client
- Conduct home visits to actively support clients throughout their pre and post-treatment journey, hospital discharge, natural aging process, and respite care
- Maintain an on-going caseload of seniors seeing minimum 4-5 clients per day
- Provide comprehensive case management that is timely, effective, efficient, equitable and client-centered
- Provide information, referrals and linkages with healthcare and other support services
- Provide follow-up to ensure that client is connected with support services
- Provide advocacy and act as a liaison with other service providers
- Design virtual and group session programs and activities promoting health and well-being Ensure client’s physical safety and emotional well-being as well as all Social Determinants of Health
- Ensure compliance with all health and safety policies
- Build collaborations with community partners and stakeholders, specifically local Hospitals, local Community Health Centre’s, local Health Service Providers, and Ontario Health at Home, etc.
- Serve as a liaison between program clients, participants and community networks
- Maintain data and submit as required
- Maintain confidentiality of client information
- Work with the Seniors Program team in achieving successful outcomes
- Plan, organize and facilitate group workshops and orientation sessions that support seniors’ health and autonomy
- Plan, prepare and evaluate appropriate material for group facilitation & workshops in collaboration with other teams and programs within the agency
- Screen, shortlist and recruit volunteers and student internship candidates
- Provide orientation and training for all accepted candidates
- Responsible for ensuring that volunteers/intern learning goals and professional experience needs are met
- Input data into appropriate database, maintains required program records, and submits required weekly and monthly reports and as requested by the management of the agency
- Develop monthly work plan and activities
- Develop individualized outreach plans for each assigned service locations in collaboration with other TNO programs or external service providers
- Jointly with service partners’ conduct programs promotion in community social landmarks
- Coordinate activities with other TNO departments and services
- Actively participates in program’s community development activities including advocacy, outreach and community engagement and empowerment activities
- Participate in networks of neighbourhood-based service providers
Screening – A clear Vulnerable Sector Check (VSC) is required
Job Type: Full-time permanent (35 hours/week) position.
Work Location(s): Teasdale/Taylor Massey and all TNO locations as required by program needs.
