Community Roots Support Services continues to expand to meet community needs, and we are excited to add to our amazing team supporting in the New Beginnings Transitional Housing Program in that serves CLBC funded individuals in Sooke, BC.
We are seeking a Community Support Worker who is organized, outgoing, creative, and grounded in person-centered practice to support adults who live independently through the New Beginnings Transitional Housing program. This position will be be full-time, up to 37.5 hours a week.
Many individuals supported through this program require assistance accessing resources in and around Victoria; therefore, a reliable vehicle and ability to transport individuals is an asset. Flexibility to work some evenings and weekends is preferred, though not a requirement.

About Community Roots:
Our focus at Community Roots Support Services is client centered; we work as a team to help provide our clients with the best support in helping achieve their goals and improve their overall quality of life.
As a Community Support Worker with Community Roots you will provide one-on-one and group support to adults with developmental and intellectual disabilities. Support given is based on the individual’s needs and the goals they have set, with a large focus on independent living skills. Areas of support include, but are not limited to, community inclusion, education, employment, self-care, health & well-being, recreational activities, social activities and development of life skills. You will receive direction and support from our Housing Coordinator, as well as peers.
Key Duties and Responsibilities:
- Works alongside the CSW's and Program Coordinator responsible for independent housing in order to ensure clients are able to engage in both group activities as well and 1-1 support as needed.
- Participates in assessment, goal setting and program planning such as Personal Service Plans for individuals living independently. Documents and implements the plan. Provides input into the evaluation of the New Beginnings program.
- Creates and implements support strategies that will focus on independent living and community connection.
- Evaluates client needs and develops short term plans to meet such needs with the active participation of clients and their families.
- Assists clients to function more independently in their own homes and in the community.
- Assists clients with daily life skills, social skills and/or behaviour management. Teaches and assists clients with activities such as personal hygiene, basic cooking, money management, shopping, household safety, pet care.
- Facilitates physical, recreational, educational, social and vocational activities.
Requirements:
- Diploma or certificate in a field related to social services, or 2 years of relevant experience.
- BC Criminal Record Check Clearance
- Valid Class 5 Drivers License and reliable vehicle
- Drivers Abstract Clearance
- Valid First Aid Certificate
- Food safe Certificate is an asset
