This position is located in Markdale, ON and is one day per week (7.5 hours).
_ Provide therapeutic counselling in complex areas such as, life transitions, grief and loss, mental health
_ Assess for and provide support in the treatment of mental health
_ Conduct psychosocial, cognitive, depression assessments
_ Complete suicide risk assessments, interventions and monitoring
_ Conduct resident and/or caregiver support groups
_ Provide mediation between home and resident/ family
_ Ongoing education for residents, family and staff on topics related to long-term care such as dementia, grief/loss, caregiver stress, resident-centered care
_ Active engagement with outside agencies to offer additional support or augment what already exists
_ Advocate on behalf of residents with respect to issues related to Powers of Attorney, Substitute Decision Makers, The Consent and Capacity Board and the Public Guardian and Trustee
_ Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
_ Bachelors or Masters Degree in Social Work from an accredited university
_ Demonstrated knowledge, sensitivity and expertise in the care of elderly and their families
_ Demonstrated ability to work effectively as part of an interdisciplinary team and independently
_ Clinical competence and commitment to quality resident care
_ Superior skills in advocating on behalf of vulnerable individuals
_ Calm and patient listener
_ Self-motivated and ability to manage a varied caseload
_ Excellent interpersonal skills and communication skills (written and verbal)
_ Excellent organizational, problem-solving, decision- making, conflict resolution and critical thinking abilities
_ Maintains current working knowledge of relevant legislation which includes the following: Long-Term Care Homes Act, Mental Health Act, Health Care Consent Act, Substitute Decisions Act, PHIPA
_ Demonstrated commitment to ongoing learning
_ Current Vulnerable Sector Check and clear 2 Step TB tests prior to start date
_ Professional and Commercial Liability Insurance
_ Experience in long-term care/ psychiatric / medical social work practice
_ Certificate in Gerontology or Dementia Care
_ MoCA Montreal - Cognitive Assessment trained and certified
_ Gentle Persuasive Approach
_ PIECES trained
