Position Summary
We are seeking an experienced and passionate Clinical Nurse Manager to lead our clinical operations in the home care setting. This role is responsible for ensuring exceptional client care, regulatory compliance, and strong leadership of our field staff including RNs, RPNs, and PSWs.
The ideal candidate is a hands-on leader who thrives in a fast-paced environment and is committed to delivering outstanding care in the community.
Key Responsibilities
- Provide leadership and oversight of all clinical home care services
- Ensure compliance with Ontario home care standards and best practices
- Conduct client assessments and oversee individualized care planning
- Supervise and support nursing staff and Personal Support Workers (PSWs)
- Monitor quality of care and implement continuous improvement initiatives
- Oversee staff training, performance management, and scheduling coordination
- Ensure accurate and timely documentation in electronic health systems
- Lead infection prevention and control (IPAC) practices
- Collaborate with clients, families, and community partners
- Support client intake, service planning, and program growth
Qualifications
- Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO), BScN preferred
- Minimum 3–5 years of leadership experience in home care or community healthcare
- Strong knowledge of Ontario home care regulations and compliance
- Experience managing PSWs, RPNs, and community-based staff
- Valid driver’s license and access to a reliable vehicle
- Experience with home care software (e.g., AlayaCare) is an asset
