Job Overview
We are seeking a highly skilled, compassionate, and dedicated Registered Practical Nurse (RPN) to join our multidisciplinary healthcare team. The ideal candidate will possess a robust clinical background across diverse care environments, including long-term care, acute hospital settings, and home healthcare. In this role, you will be responsible for delivering high-quality, patient-centered care, executing clinical procedures, and advocating for the health and well-being of a diverse patient population.
Key Responsibilities
- Clinical Excellence: Deliver comprehensive, evidence-based nursing care in strict accordance with established professional standards, regulatory guidelines, and organizational protocols.
- Medication Management: Safely administer medications and treatments, diligently monitoring, documenting, and reporting patient responses and therapeutic outcomes.
- Patient Support: Assist patients with activities of daily living (ADLs), including personal hygiene and mobility support, ensuring dignity and comfort at all times.
- Care Coordination: Collaborate effectively with physicians, therapists, and the broader healthcare team to formulate, implement, and optimize individualized care plans.
- Documentation: Maintain meticulous, accurate, and timely patient records utilizing Electronic Health Record (EHR) platforms (e.g., Epic, Cerner).
- Health Education: Educate patients and their families regarding chronic disease management, medication adherence, and post-discharge care instructions.
- Specialized Care Support: Provide specialized interventions tailored to specific patient needs, including dementia care, geriatrics, palliative/hospice care, and pain management.
Qualifications & Experience
- Professional Registration: Active registration in good standing as a Registered Practical Nurse (RPN) with the provincial regulatory body.
- Clinical Background: Previous experience in long-term care, acute care, or community/home health settings is highly desirable.
- Specialized Knowledge: Proficiency in medical terminology, advanced patient care procedures, and modern pain management techniques. Specialized experience in senior care, pediatrics, or hospice care is considered a strong asset.
- Technical Skills: Familiarity with Electronic Health Record systems (Epic or Cerner preferred); strong technical aptitude to adapt to clinical software.
- Core Competencies: Exceptional interpersonal and communication skills, a proven ability to collaborate within a fast-paced team environment, and a steadfast commitment to patient-centered advocacy.
