You will work to full scope of practice in a five Physician Family Practice Clinic in downtown Hamilton. Southwest is a patient-centered model of care with a team of highly motivated allied health care professionals including Registered Dietitian, Mental Health Counselors, Psychiatrist and Pharmacist. You will provide clinical assessments and treatment of primary care patients and participate in chronic disease management programs and patient education. This position requires excellent health assessment/clinical skills.
Experience in primary care is required. Proficiency with Practice Solution Software is required.
Pay: $36.00-$38.00 per hour
