Position Summary
We are seeking a dedicated and compassionate Community Disability Practitioner (CDP) to provide person-centered support to individuals with diverse and complex needs. This role involves supporting individuals in their homes and communities while promoting independence, dignity, inclusion, and overall well-being.
The successful candidate will work as part of a team of approximately 20 staff members and provide support to individuals with developmental disabilities, mental health concerns, and complex behavioral needs.
Key Responsibilities
- Provide mental health and social support to individuals with complex needs.
- Assist with personal care and activities of daily living.
- Support individuals with medical appointments and community participation.
- Provide transportation and community access support.
- Utilize Nonviolent Crisis Intervention (NVCI) techniques when required.
- Support communication through sign language and communication software/tools.
- Assist with household maintenance, cleaning, and daily routines.
- Monitor health, safety, and well-being while maintaining professional and respectful relationships.
Individuals Supported May Present With
- Schizophrenia
- Obsessive-Compulsive Disorder (OCD) tendencies
- Hoarding tendencies
- Autism Spectrum Disorder (ASD)
- Down Syndrome
- Physical and verbal aggression
- Non-verbal communication needs
What to Expect
This role supports individuals who require ongoing mental health, behavioral, and social support. Responsibilities may include:
- Supporting individuals with personal care
- Managing situations involving physical and verbal aggression
- Working with non-verbal individuals
- Using communication software and American Sign Language (ASL)
- Assisting with home maintenance and cleaning tasks
- Supporting activities of daily living
- Attending medical appointments
- Providing close supervision and ongoing monitoring to ensure health and safety
Required Qualifications
- Valid driver's license and reliable driving ability
- Ability and willingness to participate in swimming activities
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively within a team environment
- Commitment to person-centered support and inclusion
Preferred Qualifications
- CPI (Crisis Prevention Intervention) Certification
- Mental Health First Aid Certification
- Suicide Intervention/Suicide First Aid Training
- Experience supporting individuals with Autism Spectrum Disorder
- Experience supporting individuals with schizophrenia
- Experience working with seniors and older adults
- Experience supporting individuals with Down Syndrome
- Experience working with non-verbal individuals
- Knowledge of ASL and alternative communication methods
