- Acts as the primary point of contact for Veterans and their families with presenting or potential issues/concerns, including mental health issues, both in person and by phone, to discuss existing and potential barriers; and identify their needs through the screening process using departmentally-approved tools.
- Determines eligibility for programs or services by conducting comprehensive interviews, compiling and analyzing information from internal/external sources, assessing information gathered (e.g., verifying financial or medical requirements/information), and rendering decisions for VAC programs and services as per delegated financial authority.
- Provides guided support to Veterans by coordinating and integrating services and assisting them with navigating VAC’s systems and processes and transitioning to post-military life.
- Makes appropriate referrals for potential case management services and/or local community and provincial programs and services; provides contact information for and/or liaises with external organizations and other levels of government on their behalf to facilitate the provision of, or access to, benefits and services, as needed.
- Collaborates and consults with various members, and serves as a member, of the Interdisciplinary Team (IDT) to resolve issues for Veterans with moderate needs; and liaises with other VAC subject matter experts, as required for informed decision-making.
- Provides information and advice with respect to Veteran’s eligibility for internal/external programs and benefits; initiates applications and assessments (e.g., nursing, physio- and occupational therapy) for VAC programs and services, as required; and follows-up with their recommendations.
- Monitors and follows-up on client cases/referrals to ensure the Veteran’s needs are being met and ensures that standards are adhered to when delivering service and documenting personal information.
- Represents the Department to establish and foster partnerships with the community, private and public, including other government departments and organizations, and performs outreach activities.

Asset Qualifications :
Graduation from a recognized post-secondary institution with a degree specializing in the assessment of human behavior.
Successful completion of post-secondary school diploma (includes community college, CEGEP, technical college or university) with specialization in a health- related, human services, or social work field.
Experience as a member of the Canadian Armed Forces.
Experience in dealing with the Canadian Armed Forces military culture.
Experience in working with clients who have mental health and addictions issues.
