Based in Spruce Grove, WestView Primary Care Network is Alberta’s 2nd Primary Care Network (PCN). Working collaboratively with family physicians the PCN strengthens primary health care delivery in the communities of Spruce Grove, Stony Plain, and Parkland County.

We are currently recruiting a Behavioral Health Consultant (BHC) who will provide comprehensive functional assessment services and care to patients within the Primary Care Network. The BHC will assess, plan, implement and evaluate behavioral based treatment plans to patients of all ages. The BHC will be an integrated member working to their full scope of practice within the primary care setting.

The BHC will work directly with family physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

Key Responsibilities

To work collaboratively in a patient centered, integrated team based primary care * To participate in the care of individual patients, practice populations and associated communities. * To take on a solution-focused, goal-directed approach with clinic patients * To assist in development of brief intervention strategies with clinic patients * To make professional, competent and autonomous decisions with patients as a member of the multidisciplinary team. * To act as a resource to other multidisciplinary team members, healthcare institutions and community agencies or programs. * To ensure accurate, objective and timely clinical documentation in the appropriate medical record.

Competencies & Behaviours

  • Master’s Degree in Social Work, Nursing, Psychology or equivalent (eg. Bachelor’s Degree, RN or RPN with extensive counselling experience) * Registration with relevant College as recognized by the Health Professions Act * Current Basic Life Support (BLS) for Healthcare provider * Vehicle/license

Previous experience

  • Primary care setting * Electronic Medical Record * Group facilitation/instruction * Health behaviour change methods (eg Motivational Interviewing) * Understand the Alberta privacy laws and act according to those laws. * Should behave responsibly, be accountable and have excellent written and verbal skills, problem solving/critical thinking skills. * To support the vision and mission of the Primary Care Network.

The Behavioural Health Consultant position is full time 8:00-4:45 (38.75 hours/week – 01.0 FTE).
