Job Summary
The DUE pharmacist initiates, oversees and promotes safe, efficacious and cost effective use of medications in the provision of patient care. Through the collection, analysis, and dissemination of medication-use data, DUE services guide the development and maintenance of institutional medication policy and best practice. DUE services help to guide and inform key groups such as policy-makers, practice leaders, clinicians, committees, and administrators by providing advice based on unbiased, accurate information. The DUE pharmacist provides support and leadership to staff pharmacists in clinical and educational activities.
Working collaboratively with our integrated team of pharmacists, nurses, physicians and health system administrators, the DUE pharmacist develops and implements a structured, continuous program to assess and improve all aspects of quality of drug use. The DUE pharmacist will be a member of the corporate Medication Safety Committee, the Drugs and Therapeutics Committee and the IV Pump Committee. They will also participate in the departmental systems group.
Full training and orientation to the department will be provided in order to support all pharmacist functions if required.
Qualifications
- Licensed (Part A) with Ontario College of Pharmacists
- Membership with the Canadian Society of Hospital Pharmacists (CSHP)
- 5 years current hospital pharmacy practice experience required (may include hospital Pharmacy residency time)
- Demonstrates excellence in communication, both written and verbal
- Analysis / problem assessment

- Demonstrates individual leadership skills
