Position Summary
We are seeking a Registered Social Worker (RSW) who will provide case management, family support, and care coordination services to children and youth (up to 18 years of age) and their families enrolled in the Ontario Autism Program. This role supports families in navigating services, building capacity, and accessing resources while working collaboratively with an interdisciplinary team.
Key Responsibilities
- Conduct ongoing assessments to identify individual needs, strengths, and priorities for assigned OAP caseload
- Participate in the development, implementation, and monitoring of individualized service plans (e.g., ISPs, IEPs, care plans)
- Ensure services align with OAP guidelines, organizational policies, and individual goals
- Coordinate services across internal programs and external service providers Monitor service delivery to ensure quality, timeliness, and effectiveness
- Maintain regular communication with individuals, families, and service providers to review progress and address concerns
- Maintain accurate, timely, and confidential OAP documentation in accordance with organizational policies and regulatory requirements
- Complete required assessments, progress notes, service authorizations, and reports
- Participate in team meetings, case conferences, and supervision
- Communicate effectively with families, guardians, service providers, and community partners
- Share relevant information with supervisors and team members as appropriate
- Support individuals in crisis, including implementation of approved physical intervention strategies when required
- Perform other duties as assigned
Qualifications
- Bachelor’s or Master’s degree in social work (BSW or MSW)
- Registration in good standing with the OCSWSSW (RSW required)
- Experience working with children, youth, and families, preferably within autism or developmental services
- Knowledge of the Ontario Autism Program (OAP) considered a strong asset
- Strong communication, documentation, and organizational skills
- Ability to work collaboratively within an interdisciplinary team
- CPR/First Aid certification (or willingness to obtain)
- CPI (non-violent crisis intervention) certification (or willing to obtain)
