Queen Square Family Health Team (QSFHT) and our community partners provide accessible, equitable, patient-centred primary care to its local community. A talented, interdisciplinary team works collaboratively with Queen Square Doctors and partnering clinics, and together, we serve over 35,000 patients annually. This position is being recruited as part of our ongoing program expansion to enhance access to primary care services within the community. As part of this role, the successful candidate will be required to provide care not only at the primary site but also at external clinic locations within a 10 km radius of the main office. Travel between sites is required.
We are seeking a full-time Registered Practical Nurse (RPN) to support patient care delivery and the implementation of programs that optimize patient health. Reporting to the Manager of Nursing Services, this position will work alongside family physicians and support various nursing programs, including diabetes care, immunizations, and general patient procedures within the RPN scope of practice. Responsibilities
The QSFHT nursing team provides holistic, evidence-based, culturally appropriate, patient-centred care in accordance with the College of Nurses of Ontario standards. As a Registered Practical Nurse, you will:
- Organize patient care activities while working collaboratively with patients, families, and the inter-professional team
- Collaborate with appropriate team members regarding patient care needs
- Provide health education using effective teaching strategies tailored to patient/family needs, readiness, and motivation
- Establish and maintain respectful, therapeutic, and inter-professional relationships across the FHT and broader circle of care
- Maintain confidentiality and protect sensitive patient and organizational information
- Embrace change that improves nursing practice and patient outcomes
- Perform other related duties as assigned by the Manager of Nursing Services, Medical Director, and/or CEO
As a member of QSFHT, you will:

- Contribute to the overall success of the Family Health Team
- Participate in quality improvement initiatives, including data collection, reporting, and evaluation

Qualifications
- RPN license in good standing with the College of Nurses of Ontario
- Recent related nursing experience; preference for two years in primary care or hospital-based settings
- Current BCLS/CPR/First Aid certification
- Proof of professional liability insurance
- Experience working within an interdisciplinary team
- Comfortable with electronic medical records and technology
