Position Summary
The Coordinator facilitates thorough oversight of the care continuum for patients undergoing primary unilateral hip/knee total joint replacement. The Coordinator will act as the primary contact and resource for patients undergoing total joint replacement and for care providers to coordinate timely access to care and ensure patients’ needs are met at care transitions. This position will facilitate communication between patients, families and members of the healthcare team, advocate for individual needs of the patient, provide evidence-supported education and/or connect patients to other members of the healthcare team, as required, and support data collection and reporting related to patient and program outcomes. The Coordinator role may expand to other bundled care populations in the future.
Required
- Bachelor’s degree in a health-related discipline.
- Current registration as a regulated health professional in Ontario.
- Knowledge and experience working with patients with musculoskeletal conditions, particularly total joint replacement.
- Minimum of three (3) years of experience working with patients across total joint replacement care continuum.
- Experience collecting, tracking and reporting data.
- Knowledge of clinical budgeting principles.
- Understanding of legislation, regulations related to post-acute care Including OHIP publicly funded services, community based, convalescent care and home care services.
- Understanding of PHIPPA legislation and demonstrated ability to maintain confidentiality and exercise discretion.
- Demonstrated ability to work independently and collaboratively within an interprofessional team.
- Ability to educate patients and families regarding care plans and best practices.
- Excellent communication, interpersonal, and customer service skills.
- Strong organizational, problem-solving, and conflict resolution skills.
- Ability to build and maintain effective professional partnerships.
- Demonstrated commitment to patient- and family-centred care and responsiveness to diverse populations.
Duties
Patient Care Liaison:
- Acts as primary contact and liaison between patients, families and hospital physicians and staff with bundled care partners within and outside of the Southeast region.
- Builds effective working relationships with patients, families and care providers.
- Uses appropriate mechanisms to locate and access current and relevant information to assist the team in planning patient care.
- Engages patients and families to discuss care options and patient preferences and needs.
- Uses a patient centered approach to identify factors and risks that might present barriers to care across the care continuum.
- Advocates for the individual needs of patients and supports patient choice in health care decision-making within the performance framework of bundled care.
- Ensures patients and families have information and resources needed to make informed decisions and be active participants in their care across the care continuum, especially at care transitions.
- Connects patients to healthcare team and / or other resources, as required.
- Supports patients to take ownership of their care throughout their care journey.
Data Collection and Reporting:
- Documents and maintains accurate records to support ongoing evaluation of patient and program outcomes.
- Facilitates and/or supports the collection of patient-level administrative and patient-reported outcome data, including Patient Reported Outcomes Measures (PROMs) and additional mandatory reporting requirements, as needed.
- Submits data reporting related to patients undergoing total joint replacement and additional program outcomes, as required.
- Reports performance data and improvements at relevant committees.
Financial Accountabilities:
- Reconciles incoming and outgoing post-acute rehab invoices.
- Completes and submits Ministry financial reporting templates.
Education and Interprofessional Effectiveness:
- Works in close collaboration with all members of the interprofessional healthcare team to facilitate a team-based approach to patient care.
- Develops professional relationships with healthcare team members and provides information about bundled care Coordinators’ role and scope.
- Seeks and maintains current information about available resources for patients and families, in both the acute care setting and community.
- Assists in promoting and sustaining continuous quality improvement initiatives to drive change.
- Communicates regularly with the healthcare team to collaboratively improve transition processes.
- Provides provincial bundled care updates as necessary to key stakeholders,
- Other duties as assigned.
