Responsibilities
- Conduct thorough patient assessments and physical examinations.
- Diagnose and manage acute and chronic health conditions.
- Develop and implement individualized treatment plans based on patient needs.
- Perform procedures such as phlebotomy as needed.
- Provide case management services to coordinate care for patients.
- Educate patients and their families about health conditions, treatment options, and preventive care.
- Maintain accurate and up-to-date medical records using Cerner or similar electronic health record systems.
- Collaborate with interdisciplinary teams in nursing homes and other healthcare settings to ensure optimal patient outcomes.
- Participate in pain management strategies for patients requiring specialized care.
- Stay current with best practices in nursing and healthcare trends.
Requirements
- Master’s degree in Nursing or a related field from an accredited program.
- Valid Nurse Practitioner license in the state of practice.
- Strong clinical skills with experience in pediatrics, senior care, or nursing home settings preferred.
- Proficiency in case management practices and the ability to work effectively within a medical office environment.
- Experience working with children is a plus.
- Familiarity with electronic health record systems, particularly Cerner, is advantageous.
- Excellent communication skills and the ability to build rapport with patients of all ages.
- Strong organizational skills and attention to detail.
