Licensed Practical Nurse (LPN) Supervisor – Continuing Care / Home Care
Location: Edmonton and Surrounding Areas, Alberta
Job Type: Full-Time, Permanent
Schedule: Rotating shifts (days and evenings, primarily evenings) | On-call alternate weekends

About the Role
Reporting to the RN Manager, our LPN Supervisor provides hands-on clinical leadership, supports after-hours and weekend coverage, and plays a key part in welcoming new clients into our care. You'll guide our care staff, build real relationships with the seniors and families we serve, and help set the standard for the quality of care we're known for.
Key Responsibilities
- Meet with clients and conduct pre-admission meetings and assessments
- Perform nursing assessments and identify clients' health care needs
- Develop and update care plans based on assessment findings
- Provide supervision and clinical support to care staff
- Participate in an on-call rotation covering alternate weekends
- Work a rotating schedule of day and evening shifts (mostly evenings)
- Communicate effectively with clients, families, staff, and the care team
Qualifications
- Current LPN license in good standing with the College of Licensed Practical Nurses of Alberta (CLPNA)
- Minimum 5 years of continuing care experience (required)
- Home care experience an asset
- Strong nursing assessment skills with the ability to identify client health care needs
- Excellent communication and interpersonal skills
- Valid driver's license and reliable vehicle (travel to client homes required)
Schedule
- Full-time, rotating shifts: day and evening (primarily evenings)
- On-call coverage on alternate weekends
