We're seeking dedicated nurses who want to provide high-quality, one-on-one care to a single client. This role provides the stability and consistency often missing from hospital work, with a focus on building a strong rapport with your patient and their family.
Schedule will be:
Night shifts (2200-0600 or 2300 to 0700)
What You’ll Be Doing:
· Deliver comprehensive nursing care to patients in home settings, focusing on individualized treatment plans.
· Conduct thorough assessments of patients' health status and develop appropriate nursing interventions.
· Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services.
· Educate patients and their families on health management, medication administration, and home care practices.
· Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
· Maintain accurate documentation of patient care activities using electronic health record systems, such as Epic.
· Provide support for laboratory tests and procedures as needed, ensuring adherence to safety protocols.
What we look for in our candidates:
· Previous experience with pediatrics is highly desirable.
· Experience with Trach care, CPAP, BiPAP, and home ventilators.
· Strong background in patient care with a focus on community home health.
· Excellent communication skills and the ability to work collaboratively with healthcare teams and families.
· Experience working with children and understanding their unique healthcare needs is beneficial.

Pay: $33.00-$36.00 per hour
