Placement Care Manager

As the Placement Care Manager, you would be responsible and accountable for providing direction, team leadership and organization of the Centralized Placement Team in consideration of AHS business plan, performance measures and team goals. In this position, you will be required to develop strong working relationships with key stakeholders, promote team development, assist with change management, and education on policy, process and services. You will also be involved in committee meetings/working groups as assigned to represent the North Zone Centralized Placement Team. As the Placement Care Manager, you will review complex cases and support the team with communication, recommendations or attendance at family conferences to facilitate positive outcomes. You will facilitate weekly team meetings to review transitions, complex cases, vacancy management, waitlists and work collaboratively to enhance the client’s transition through the continuum of care. Responsibilities will include the promotion of collaborative care, patient first strategies and maintaining a client/family centered approach throughout the process of accessing a designated living option. In this position, you will evaluate performance measures and identify opportunities for improvements. As well, assist to respond to urgent requests for information related to placement statistics, waitlists, site information or policy and process.

Required Qualifications:
Completed a bachelor’s degree in a health care profession, health management or related field. Active or eligible for registration and practice permit with the applicable college.

Additional Required Qualifications:
Three to five years’ Senior Health experience that includes home care and transition services.

Preferred Qualifications:
Experience with home care & transition services; Knowledge of computer applications (MS Office) & Meditech; Experience with RAIHC & Designated Living Option Guidelines; Experience working with interdisciplinary teams; Strong leadership abilities; Effective communicator with negotiation & conflict resolution skills; Knowledge of applicable legislation, programs/community resources; Ability to establish team goals & achieve outcomes; Ability to multitask and prioritize workload.
