We currently have a Regular Full-time opportunity for an Addictions Counsellor in the Brain Sciences - RAAM Clinic department at Bayview.
Hours of Work:
37.5 hrs/week (includes evening clinic – Monday 5-7pm)
Summary of Duties
A rapid access addictions medicine (RAAM) clinic is a low-barrier, walk-in clinic that patients can attend to get help for a substance use disorder without an appointment or formal referral. RAAM clinics provide time-limited medical addiction care (including pharmacotherapy, brief counselling, and referrals to community services). Sunnybrook RAAM is part of a network of clinics across the province. The Sunnybrook RAAM is expanding to provide three half day clinics and is recruiting new members of the inter-professional team to play a role in this exciting initiative.
The addictions counsellor is an integral member of the team, working collaboratively with physicians, nurse practitioners, counsellors, peer support workers, and administrative staff to; assess, develop treatment plans, initiate treatment, and transition people to services they may require in the community. The position is well suited to a self-starter with strong organizational and communication skills who enjoys being part of building new services, building partnerships internally and externally and working with diverse communities.
The successful candidate will;:
Complete comprehensive bio-psycho-social and addictions assessments for new clinic clients
Provide therapeutic interventions for clients, as appropriate (brief solution-focused therapy, CBT, DBT, Narrative Therapy, etc)
Provide education and support to family members around issues related to addiction/mental health
Provide time limited case management support and assisting clients in meeting their short term goals
Support clients to successfully connect with community-based addiction and mental health services or hospital programs
Participate in developing and delivering evidence based group interventions dependent on needs of client population (i.e.
SMART recovery, relapse prevention, psychoeducation, Seeking Safety)
Document client interactions in electronic medical record according to hospital policy
Do outreach to community partners to build relationships that support successful transitions to community services
Collaborate with team to identify and respond to opportunities to further integrate RAAM clinic services into hospital
Provide teaching to medical students and hospital staff partners
Contribute to building and expanding the addiction services at Sunnybrook Hospital
Minimum of two (2) years post-graduate counselling and case management experience delivering a variety of
interventions, with focus on addictions
Experience working in an outpatient addiction & mental health setting
Experience with addiction patients counselling/brief/short-term psychotherapy models of care, and crisis intervention
Experience with helping clients navigate and transition to other services as needed (ie primary care service, psychiatric care, etc)
Knowledge of the addictions & mental health system and community agencies
Knowledge of harm-reduction approaches and trauma-informed care
Relevant ongoing professional development (i.e. motivational interviewing, assessment skills, counselling modalities, etc)
Demonstrated clinical knowledge and experience in the area of substance use
Significant knowledge and experience in varieties of counseling models (brief solution-focused therapy, CBT, DBT, Narrative Therapy, etc)
Ability to do addiction assessments independently and provide comprehensive treatment planning and treatment to clients with addiction issues
Knowledge and understanding of diverse cultures and communities required.
Experience in keeping accurate clinical, operational and statistical information.
Experience and understanding of working with family dynamics.
Experience working with clients with concurrent disorders
Ability to work independently as well as, collaborate with a multi-disciplinary team
Demonstrated and effective organizational, interpersonal and communication skills (written & oral)
