ONE CARE Home and Community Support Services is looking for a highly motivated individual to join our team in a full time capacity for a 12 month contract. This role is responsible for scheduling each client with an appropriate Personal Support Worker (PSW) in order to provide the services outlined in the client care plan. This is an in person role with the potential for a hybrid schedule after completion of training.
Hours Of Work: Days, evenings, every other weekend (includes 6:00 am-2:00 pm/7:00 - 3:00 , 8:00 - 4:00 , 1:00-9:00 pm shifts).
RESPONSIBILITIES:
- Intake of clients regarding scheduling and data entry into our Client management System.
- Schedule the work of the PSW while taking in to consideration the time, client needs, PSW availability and their strengths/skill set to find the best possible match.
- Consult with the In-Home Care Supervisor to determine the appropriate match between client and PSW.
- Monitor Health Partner Gateway (HPG) for referrals and other health information from community partners.
- Accountable for carrying pager outside of normal business hours but within your scheduled shift. To contact a Manager/Supervisor for non-scheduling issues.
QUALIFICATIONS:
- Post Secondary Diploma in Office Management or equivalent is preferred.
- Knowledge of medical terminology and documentation.
- 2 – 5 years scheduling experience an asset.
- Demonstrated problem solving, ability to assess situations, set priorities and decision making skills.
- Demonstrated willingness and ability to be adaptable, build relationships, focus on clients needs and communicate effectively.
- Excellent interpersonal, communication, computer and time management skills.
- Valid Ontario driver’s license and proof of insurance.
- French Speaking would be considered an asset.
