Case Manager – Geriatric Psychiatry
Geriatric Psychiatry Community Services of Ottawa (GPCSO) is a community-based mental health program funded by Ontario Health serving older adults in the Ottawa region. Case Managers provide comprehensive assessment, treatment and follow-up to seniors with mental health problems and/or dementia with responsive behaviours, as well as support and education to caregivers. Case Managers are Registered Nurses, Occupational Therapists, or Social Workers.
Responsibilities
· Conducts a comprehensive multi-dimensional psychosocial assessment of clients in their homes and develops a person-centred treatment plan.
· Delivers interventions to:
- Support self-management of mental health symptoms,
- Understand and reduce behavioural expressions in persons living with dementia,
- Enhance caregiver understanding of approaches to care that support meaningful engagement,
- Characterize and communicate risk and collaborate with clients, family members and health care partners to understand options, implement decisions and balance risk versus autonomy,
- Make referrals to community resources and other health care agencies to support client goals.
· Works closely with the geriatric psychiatrist in the provision of high quality mental healthcare
· Collaborates with community partners to support increased capacity to care for older adults living with dementia and/or psychiatric conditions in the community.
· Communicates clearly and effectively with Primary Care Practitioners and referral sources
Qualifications
Occupational Therapist with a Bachelor or Master's degree or Social Worker with a Master's degree or Registered Nurse
- At least three years’ experience in geriatric psychiatry or related field
- Member in good standing in a professional college (CNO, COTO, or OCSW&SSW)
- Comprehensive knowledge of psychiatric conditions affecting older adults
- Knowledge of community resources
- Excellent written and verbal communication skills
- Driver’s license and access to a reliable automobile
- Bilingualism strong asset
