H.J. McFarland Memorial Home in idyllic Prince Edward County is accepting applications from Registered Nurses for the position of RN: Care Coordinator. Reporting to the Director of Care, the Care Coordinator is responsible for providing the highest level of nursing care in accordance with the College of Nurses of Ontario and RNAO Best Practice Guidelines, physician orders and administrative policies.
Working Monday to Friday, the Care Coordinator prepares, implements, reviews and evaluates all resident care plans and monitors summaries, residents’ health, notes changes in conditions, and assesses the need for doctor referrals. The Coordinator provides advice on resident care needs in various settings, leads the Falls and Restraints program, participates in the medication management program, and acts as backup for the RAI Coordinator and the IPAC Lead when the incumbents are away.
The Care Coordinator is accountable for working collaboratively with the other nursing team members to provide quality resident care, ensuring resident safety by following relevant acts, regulations, professional bodies, policies, and directives from the Ministry of Health, Long-Term Care and CARF Accreditation, and for supporting the achievement of the goals and objectives as set out in the Mission, Vision and Values of the Home.
WHAT YOU BRING TO US
- Current unrestricted registration as an RN with the College of Nurses of Ontario, required.
- Post-secondary diploma or degree in Nursing.
- Minimum one year of Nursing experience required, preferably in a long-term care setting.
- IPAC certification, or willingness and ability to obtain within six (6) months, required.
- RAI experience, an asset.
- Excellent interpersonal skills with the ability to effectively communicate with residents, family members and staff and to function effectively as part of a multi-disciplinary team.
- Ability to work with minimal supervision, including strong organizational, time management, administrative, and decision-making skills.
- Ability to prioritize and work efficiently and accurately to meet deadlines in a fast-paced environment, responding with flexibility to changing priorities.
- Advanced computer proficiency with Microsoft Office applications, Point Click Care and other computerized nursing documentation programs.
- Current satisfactory Vulnerable Sector Check.
WHAT WE PROVIDE TO YOU
- $42.00 to $56.95 per hour dependent on a credit for experience check, as per the ONA Collective Agreement
- $25,000 from Ontario Health if you qualify and commit to H.J. McFarland for 24-months (see below for details)
