About Musqueam
We are traditional hən̓q̓əmin̓əm̓ speaking people. Today, we are a strong, growing community of over 1,300 members. We live on a small portion of our traditional territory, known as the Musqueam Indian Reserve, located south of Marine Drive near the mouth of the Fraser River. The name Musqueam relates back to the River Plant, the name of the plant is məθkʷəy̓. There is a story that has been passed on from generation to generation that explains how we became known as the xʷməθkʷəy̓əm (Musqueam) - People of the River Plant. We have always moved throughout our territory using the resources it provides for fishing, hunting, trapping and gathering. We remain distinct and our cultural practices are strong, despite the devastating impacts of residential schools, colonial laws banning our ceremonies, and other attempts to assimilate our people. Our lands and waters continue to support our cultural and economic practices while serving as a source of knowledge and memory, encoded with our teachings and laws. Position Summary
Reporting to the Health Director, the Community Health Nurse (CHN) is responsible for providing a comprehensive range of nursing functions and services within the Musqueam community in the program areas of community and public health, through promotion and self-management education of the health of individuals, families, and community with a focus on promoting health, preventing disease and injury and protecting population health. The CHN facilitates the planning of activities and strategies to support the community in achieving healthy outcomes. This includes participating in the design and delivery of community health and educational programs that address health promotion from preconception to end-of-life for all Musqueam community.
Community health programs may include, but are not limited to, Maternal and Child Health, Child and Youth Health, Adult and Elder Health, Sexual Health, Chronic Disease Prevention and Management, Health Promotion, Environmental Health, Mental Wellness, Harm Reduction, and Community Wellness initiatives. The CHN supports a holistic approach to health and wellness that honors Musqueam values, culture, traditions, and community priorities while promoting equitable access to healthcare services and resources.
Roles & Responsibilities
- Plans, coordinates and delivers routine Immunization and Communicable Disease Control program services
- Performs case monitoring and outbreak control functions as a member of an integrated health team.
- Working in the Musqueam community with interdisciplinary and departmental teams to update community health plans and programs.
- Planning programs, community events, activities and strategies to support the community in achieving healthy outcomes and participates in the design and delivery of community health programs (including educational programs & workshops), that address health promotion, illness prevention and health protection across the lifespan.
- Responsible for maternal child health education and regular follow-up with families; hosting baby and me sessions, chronic disease management; health promotion and disease prevention to the community as a whole; communicable disease prevention and communicable disease follow-up; mental health promotion; community nutrition promotion and family health promotion.
- Supervises teams in the delivery of community health programs and serves as a community level health resource.
- Advocating for access to resources and other community services, programs and funding.
- Maintains current knowledge of programs and services available through partners.
- Promotes optimal wellness for individuals suffering from chronic illness through monitoring, education and counseling in partnership with other community health care providers.
- Supports clients in their pursuit of culturally safe health care from external providers; and makes client referrals to care providers, hospitals and services.
- Carries out community health events [e.g. Women’s Health Fair, Men’s and Women’s Health, Community Health Surveys].
- Collects and compiles event data to prepare analyses and reports.
- Upholds and maintains nursing practices through in-house and external professional meetings, in-services and conferences, and review of professional literature
- Health promotion and primary prevention supporting wide range of community-led and culturally relevant health promotion and prevention activities to promote diabetes awareness, healthy eating, physical activity and healthy lifestyles
- Other duties as required.
Knowledge, Skills and Abilities
- Bachelor of Science, Nursing (BSN) degree, current practicing membership in good standing with the College of Registered Nurses of B.C., and/or eligibility for licensure with the BCCNM.
- Current HCP/CPR/AED Level C certification and current Immunization certification.
- Minimum two (2) years' experience working as a Community Health Nurse in First Nations context is preferred.
- Understanding of diversity, equity, inclusion, belonging, cultural safety, and anti-racism principles, with the ability to provide equitable, respectful, and culturally responsive services that honor Indigenous perspectives, identities, and lived experiences.
- Knowledge of harm reduction principles and practices, including evidence-based approaches that support the health, dignity, safety, and self-determination of individuals and families.
- Experience in diabetes education is an asset.
- Demonstrated ability to exercise sound clinical judgment and provide independent nursing care within a community-based setting.
- Ability to remain calm, adaptable, and professional when responding to medical emergencies, falls, crisis situations, or other unexpected events.
- Analytical and problem-solving skills to carry out community health assessments, identify and recommend/develop interventions to address multi-faceted community/public health problems and provide professional nursing advice.
- Proficient in Microsoft Office and electronic medical records or other relevant health information systems.
- Excellent verbal communication and public speaking skills, with the ability to present complex health information in an accessible, culturally safe, and engaging way to individuals, families, community and clinicians.
- Successful Criminal Record Check.
Working Conditions
- Work is performed in an office environment and out in the community.
- After hours work may be occasionally required due to program demands and/or in the event of a communicable disease.
- Valid Driver’s License required with access to a vehicle for work purposes.
