PURPOSE
The development of the UHN at Home Program is a key priority for UHN to deliver on the strategic objective of Integrated Care and Older Adult Strategy. This role requires working across all clinical and administrative areas at UHN and with key community and health system stakeholders to develop and support sustainable operations of UHN at Home to transform care delivery and the patient and caregiver experience in the community.
UHN at Home and Integrated Care Strategy: Contribute to the development of a strategy to deliver evidence informed, best-practice models in the community, in collaboration with all programs at UHN; Collaborate with the VP UHN at Home, Director, Integrated Care and UHN Clinical Program Leaders to develop and implement a UHN at Home strategic plan that supports the Integrated Care strategy and expansion of Integrated across UHN and into the community.
UHN at Home Development and Innovation: Provide leadership, direction, and coaching to multiple teams responsible for leading, developing and implementing a new model of home and community care, by leveraging innovative partnership models with community partners and teams who have been trained for home and community care of the future. This role is accountable for leading development of new roles in the community, innovative contracting models, and specialized education and upskilling of staff (in partnership with the Michener Institute). Will develop systems to monitor performance of the model by supporting earlier, safe discharges from acute care, reducing readmissions and ED visits and improving the overall experience of care for patients, caregivers and frontline providers.
Clinical Delivery and Operations: Provide oversight for the core functions of Home Care operations, provided through contracted providers and direct service delivery. The core functions include utilization management, scheduling of staff, an integrated clinical record and other functions required to support a seamless and coordinated home based program.
Care Coordination and Navigation: Lead the development of the Care Coordination model for UHN at Home and Integrated Care; this includes development of a staffing model for care coordination, collaborating with other system partners to align care coordination services for Integrated Care and oversight of care coordinators (IC Leads and other relevant roles) who perform the functions of care coordination for Integrated Care. regionally and provincially.
Responsibilities:
Development and Implementation of the UHN at Home
Performs corporate management responsibilities that support and advance UHN’s goals.
Performs planning responsibilities.
Manages all aspect of human resources including contracted services
Performs quality management responsibilities.
Manages the unit/department financial resources.
Performs cross-functional and/or other duties consistent with the job classification, as assigned or requested.
Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention’s of the Act, in a timely manner.
Education:
At minimum, completion of a master’s degree in Medical Science, Business or Health Administration required.
Change management training/certification preferred
Communications education and training preferred
Experience:
At minimum between 8 – 10 years of practical and related experience and on-the-job training preferred.
Experience leading large transformation initiatives requiring engagement of diverse stakeholders across the care continuum, including patient and caregiver involvement
Experience in Home and Community sector preferred
Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
