Provide direct patient care within the RPN scope of practice and in accordance with College of Nurses of Ontario standards, organizational policies, and medical directives.
- Complete patient assessments, obtain health histories, record vital signs, and document clinical findings in the electronic medical record.
- Support preventative care initiatives including immunizations, screening programs, and health promotion activities.
- Perform medication reconciliation appointments and provide patient education regarding medications and treatment plans.
- Assist with chronic disease management programs including diabetes, hypertension, COPD, and cardiovascular disease management.
- Perform cervical cancer screening (Pap tests) following completion of required education, certification, and competency assessment.
- Administer liquid nitrogen (cryotherapy) treatments and other approved procedures following completion of required training and competency assessment.
- Assist physicians and nurse practitioners with minor procedures, treatments, and follow-up care.
- Provide wound care, point-of-care testing, specimen collection, and other nursing procedures as required.
- Participate in patient intake appointments, care coordination, follow-up activities, and transitions of care.
- Collaborate with physicians, nurse practitioners, allied health professionals, and community partners to support comprehensive patient care.
- Maintain accurate, timely, and complete documentation within the electronic medical record and participate in quality improvement initiatives and ongoing professional development activities.
- This position is Mondays and Wednesdays at Maitland Valley FHT, Goderich and Tuesdays at Bluewater Area FHT, Zurich
Pay: Up to $31.00 per hour
