POSITION TITLE: Nurse Practitioner, Most Responsible Provider (MRP)
Rideau St. Lawrence Family Health Team (RSLFHT), located in Winchester, is a not-for-profit organization offering a range of comprehensive primary health care and health promotion programs through an inter-professional care team. POSITION SUMMARY: : The Nurse Practitioner, MRP (NP) works as part of the inter-professional team with physicians, nurse practitioners, nurses, social worker, pharmacist and other staff in the delivery of high quality Primary Health Care services to RSLFHT clients. The NP will be responsible for their own patient roster and provide primary health care services within the scope of the RN-EC including physical assessments, treatment, diagnosis and interpretation of test results, therapeutics, health promotion/education/counselling, history taking, screening, referral, and follow up. The NP consults with physicians and/or other health care providers when client’s condition requires care beyond the scope of practice (RN-EC).
QUALIFICATIONS:
- Baccalaureate in Nursing from a recognized university.
- Current and valid registration in the Extended Class with the College of Nurses of Ontario.
- Experience and proficiency in current nursing physical assessment and treatment methods within the scope of practice of the Registered Nurse, Extended Class.
- Experience working in a primary health care setting or a combination of hospital and community settings and/or public health. Nursing experience working with priority populations and working within a multidisciplinary setting.
- Strong communication skills in oral and written required.
- Strong inter-professional skills required and the ability to work well within in a team.
- Demonstrated ability to work in an Interprofessional team environment;
- Demonstrated experience in teaching and providing health education to individuals and in group settings; as well as to Interprofessional learners.
- Excellent computer skills and a minimum of 2 years’ experience with an Electronic Medical Records (EMR) Primary Care system or equivalent experience required;
- Strong assessment and triaging skills, acute care experience an asset
- Knowledge and proficiency in current, evidence based methods and practices of primary care delivery
- Excellent interpersonal skills;
- Excellent decision-making, problem solving, conflict management and time management skills.
- Current Ontario Driver’s License and satisfactory police record check.
