Reporting to the Manager, the focus of this position includes the development and implementation of individualized patient treatment plan with emphasis on counselling and guidance of the recovery process. The Addiction Support Worker (ASW) works collaboratively with the treatment team in the implementation of the patient’s treatment plan and delivers substance use services, guidance, education and materials in relation to addictions and recovery. The ASW will endeavor to develop therapeutic relationships with patients and family members.
Responsibilities:
- Facilitation of groups with a focus on addiction and recovery
- Establishes therapeutic rapport through 1 on 1 session with patients to provides support, education and guidance
- Provides education to the treatment team regarding evidence informed pharmacological interventions ie naloxone, naltrexone, suboxone and sublocade
- Provides education to patients and families regarding treatment goals ie harm reduction and abstinence
- Engagement and connection with community providers to support harm reduction and abstinence goals
- Ensures accurate and timely documentation of patient records
- Maintains collaborative working relationships with other members of the health care team
- Participates in continuous quality improvement activities related to unit specific in-service programs, or education service programs
- Provides direct client care procedures and non-client care functions
- Responsible for making appropriate referrals recommendations and participate in discharge planning
- Produce and maintain monthly statistical information
- Connect clients with virtual or in person community services (AA, NA, BFO)
QUALIFICATIONS:
- 1-2 years of experience in Mental Health & Addictions-preferred
- If in recovery from substance use, must have three years of continuous abstinence
- Good knowledge of phases, progression and symptoms of chemical and alcohol dependency addictions
- Demonstrated ability to deal with addictions in a supportive, non-judgemental manner
- Proven ability to maintain accurate and appropriate case notes
- Ability to work effectively as a member of a multi-disciplinary care team and collaborative team environment
- Excellent communication skills; Proficiency in French language is an asset
Post-Secondary addictions diploma from a university or community college of recognized standing – required
Graduate of a Mental Health & Addictions Diploma Program – required
Chemical Dependency Counsellor Diploma – preferred
Certificate in CCACF – preferred
Completion of Opioid Dependence Certificate – preferred
Crisis/Suicide Intervention Training – preferred
Certificate in CPR/First Aid – required
