The Alternate Health Facility (AHF) is a 40-bed unit located off site. The AHF will care for patients who require transitional care or an alternative destination of care whose acute care needs have been met, but may require transfer to rehabilitation, long-term care, a retirement home or other post-acute care services. Patients at the AHF will be cared for by an inter professional care team of physicians, nurses and personal support workers, in conjunction with dietary, environmental services, and support from the Infection Prevention and Control Practitioner. Equipped for acute medicine patients, the AHF site offers the same quality of care as NBRHC main site.
QUALIFICATIONS
Certification/Experience/Knowledge:
- Current Certificate of Registration ~ College of Nurses of Ontario required
- Current Basic Life Support (BLS) Certificate required
- Dementia care, pharmacotherapy, falls/mobility, osteoporosis, delirium training required
- De-escalation techniques required
- Medical clinical experience in an acute care setting required
- Certified in Medication Administration required with recent related experience required
- PIECES and Gentle Persuasive Approach (GPA) Certificate an asset
- Basic computer knowledge required
Skills/Abilities:
- Excellent level of interpersonal skills required
- Excellent level of written and verbal communication skills required
- Ability to effectively provide health teaching to individuals and groups required
- Ability to work as a collaborative member of the health team required
- Ability to assume responsibility for patient care in a total patient care climate required
- Ability to work with minimal supervision required
- Ability to meet the physical demands required to provide nursing care to medical patients required
- Demonstrated ability to engage in meaningful patient activity is required
- Demonstrated ability in flexible care and patient centered approaches is required
- Critical thinking, problem solving, ability to delegate, multitask is required
- Must demonstrate commitment to ensure your safety, the safety of fellow workers, patients, volunteers, and visitors
- Must support and contribute to a culture of safety and prevention of adverse health events in this organization
- Good attendance and work record required
- Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
