We are searching for a Seniors’ Navigator to join our Nursing Home Without Walls team at Medley & Neill Hall based in Fredericton, New Brunswick (northside).
This is a temporary full-time position on a one (1) year term with potential of extension.

About the Opportunity
Support the development of and executes a community outreach program for the older population (60+) to meet the objectives of the project. The community includes the City of Fredericton and the surrounding area.
- Ensure that seniors and their families/friends/neighbors have access to appropriate services and information related to aging in place;
- Provide social health initiatives to counter social isolation and loneliness to seniors and their caregivers;
- Increase knowledge on health-related issues important to aging in place and healthy aging for seniors and their caregivers;
- Empower the local community to respond to the needs of an aging population;
- Oversee and manages the Fredericton Nursing Home Without Walls team.
Support the evaluation of how social and built environments can foster healthy aging by creating and implementing evaluation tools and reports on initiatives carried out.
- Based on the premise of the NB Home First Strategy and the NB Strategy on Aging/Family Plan provide services and support options needed for aging in place;
- Developing programs that address social isolation;
- Providing health promotion opportunities;
- Providing a supportive community environment for seniors and their caregivers;
- Disseminating information that supports aging in place.
Examine the capacity of community level approaches to address health inequalities.
- Support interventions for aging in place;
- Build capacity of social contacts so that vulnerable seniors can safely remain at home;
- Provide opportunities for members of the community to collaborate with the nursing home to support seniors in their community;
- Contribute to determining best practices for aging in place by relying on infrastructures already present and available in most communities;
- Advocate for evidence informed best practices.

- Business hours are typically 8:00 – 4:00, however, some weekends and evenings will be required.
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- College or University education in such areas as social work, gerontology, health care professionals, project management, business management, and/or health and well-being studies related to the position;
- Good knowledge of aging, age-friendly communities and resources as well as issues relating to the older population;
- Ability to write and speak English is required, French is considered a strong asset;
- Thorough understanding of performance measurement and evaluation;
- Thorough understanding of project management;
- Excellent self-motivation and problem-solving skills;
- Excellent communication skills both written and verbal and the ability to build positive relations and collaborations;
- Ability to present information to individuals and groups;
- Experience in engaging people through assessments, and/or group work is an asset.
