Your Opportunity
We are seeking a compassionate and collaborative Occupational Therapist (OT) to join our interdisciplinary Family Health Team. In this role, you will support patients in achieving optimal health, independence, and quality of life by assessing physical, cognitive, psychosocial, and environmental factors affecting a patient’s ability to perform daily activities and participate in meaningful occupations.
The OT provides individualized interventions, education, and recommendations to improve function, safety, and overall well-being while collaborating closely with physicians, nurses, and other healthcare professionals.
Key Responsibilities and Skills:
Clinical Assessment & Patient Care
- Conduct comprehensive occupational therapy assessments, including functional abilities, cognition, mobility, activities of daily living, home safety, and environmental factors.
- Develop and implement individualized treatment plans that promote functional independence, safety, and participation in meaningful activities.
- Recommend adaptive equipment, assistive devices, ergonomic solutions, and environmental modifications to optimize patient function.
- Provide patient and caregiver education on disease management, fall prevention, energy conservation, cognitive strategies, joint protection, and self-management.
Seniors' Health & Disability Management
- Conduct falls prevention, home safety, and functional assessments to support aging in place and safe transitions between hospital, community, and home.
- Complete functional assessments, Functional Ability Forms (FAFs), and provide recommendations to support disability management, workplace accommodations, and graduated return-to-work planning.
- Assess the impact of cognitive and mental health conditions on daily functioning and collaborate with the interdisciplinary team to coordinate appropriate care.
Health Promotion & Education
- Develop and facilitate individual and group education programs related to healthy aging, chronic disease management, chronic pain, falls prevention, and self-management.
Collaboration & Professional Practice
- Collaborate with the interdisciplinary healthcare team through case conferences, care planning, quality improvement initiatives, and ongoing professional development.
- Maintain accurate, timely, and complete documentation within the electronic medical record.
- Perform other related duties as assigned.
Job Requirements:
Knowledge and Skill Requirements
- Bachelor's or master's degree in Occupational Therapy from an accredited program.
- Current registration and membership in good standing with the College of Occupational Therapists of Ontario
- Minimum two (2) years of clinical experience preferred; experience in primary care, community health, rehabilitation, or Family Health Teams is an asset.
- Knowledge of occupational therapy assessments, evidence-based interventions, and community resources.
- Experience supporting individuals with chronic disease, chronic pain, mental health concerns, cognitive impairment, and functional limitations.
- Knowledge of disability management, functional assessment, and return-to-work planning is considered an asset
- Proficiency with electronic medical records (OSCAR preferred) and standard computer applications.
- Strong assessment, critical thinking, problem-solving, and communication skills.
- Ability to work effectively both independently and collaboratively within an interdisciplinary team.
- Excellent organizational, time management, and prioritization skills, with the flexibility to adapt to changing organizational needs.
- Demonstrate commitment to patient-centred care, continuous learning, and professional excellence.
