The Outreach Program is a partnership with James Bay New Horizons.
The goals of the Outreach Programs are:
· To reduce isolation of seniors
· To connect seniors to activities, community resources and services to improve their quality of life.
· To connect seniors to their neighbours
Reporting to the Executive Director(s) the Community Outreach Worker is responsible for connecting with older adults in the community and facilitating activities and information sessions at off-site housing and other locations in the community. As well as assisting and providing one-on-one support to individuals on a variety of concerns to help them remain independent.
Key Responsibilities: Outreach
· Research, plan and implement group information programs and sessions at various locations around Greater Victoria for older adult in subsidized housing and other sites.
· Coordinate schedule of sessions and promote to Property Managers, Landlords, Partners and older adults in the community.
· Set up displays and provide resource and promotional materials to the group participants.
· Have a working knowledge of a wide range of community resources to address a variety of issues. Provide one on one support as required to connect seniors to support and resources.
· Manage client information, including attendance, tracking of intake, follow up calls and keep all statistical information and confidential records.
· Maintain contact and connections with attendees after the initial contact and build relationships.
· Program planning, development and facilitation experience required. Health, Wellness and Physical activities an asset.
· Adhere to all aspects of confidentiality, privacy legislation and Safety Plans.
Education and Experience
· Degree or Diploma in Social Work, Education or other Human Service field
· Minimum of two years’ experience working with seniors with a variety of health related and other issues.
· Experience working for organizations that facilitate the well-being of older adults
· Volunteer management experience an asset
Other Requirements
· Police Information Check
· Current First Aid/CPR
· Valid driver license and own transportation
· Class 4 License would be an asset
Part time: $28 per hour. 21 hours per week.
Schedule to be determined and would be between 8:30 am to 4:00pm Monday to Friday.
