JOB DESCRIPTION
The Integrated Community Access Program is seeking a full-time Community Mental Health Nurse to provide mental health nursing services to individuals with serious mental illness- in particular those individuals diagnosed with psychosis.
As a key member of the outreach team, you will provide clinical assessment and treatment, group facilitation, consultative and educational services to clients, families, community providers, referral sources and other community partners. You will have demonstrated initiative and creative approaches to challenging situations and will be willing to work collaboratively as this program continues to work towards specialized evidence-based services in a learning environment. The successful candidate will have the opportunity to participate in service redesign and implementation of new services.
Requirements:
- The successful candidate must be registered with the College of Nurses of Ontario in good standing.
- Current CPR-HCP certification.
- Knowledge and experience working with individuals with serious mental illness and complex outpatient presentations, particularly individuals diagnosed with psychosis.
- 3–5 years of recent psychiatric nursing experience within a mental health facility or service.
- Experience providing community-based mental health services.
- Demonstrated knowledge and experience in psychiatric medication management, medication education, monitoring for therapeutic effectiveness and adverse effects, and administration of Long-Acting Injectable (LAI) medications.
- Knowledge and/or experience supporting individuals receiving treatment under Community Treatment Orders (CTOs), including an understanding of relevant requirements under Ontario's Mental Health Act.
- Demonstrated experience with transition and discharge planning, including coordination between inpatient, outpatient, primary care, and community-based services.
- Knowledge and experience related to the challenges associated with community reintegration of long-stay psychiatric inpatients.
- Ability to work effectively and collaboratively as part of an interdisciplinary team.
- Training and certification in Cognitive Behavioural Therapy (CBT) for this population.
- Demonstrated knowledge of Canadian Standards for Psychiatric-Mental Health Nursing and relevant policies and legislation, including the Mental Health Act and PHIPA.
- Maintain reflective practice standards as described by the College of Nurses of Ontario.
- Current driver's licence and access to a vehicle are required.
- Must have the ability to perform the physical demands of the position.
- BScN preferred.
- Training and/or experience facilitating psychoeducational groups.
- CNA certification in psychiatric/mental health nursing is an asset.
- Experience delivering presentations is an asset.
- Demonstrated excellent interpersonal and effective oral and written communication skills.
- Ability to work effectively with clients from diverse ethno racial and cultural backgrounds; proficiency in a second language is considered an asset.
