Diabetes Educator, Registered Nurse or Registered Dietitian
(40 hours/week, Contract to December 2027)

What We are Looking For:
We are currently recruiting a Certified Diabetes Educator to join our Diabetes Education Team in a full‑time (40 hours per week), contract position. This role is responsible for delivering diabetes education programs and services to individuals and groups living with Type 2 diabetes and prediabetes within a CHC setting. Experience supporting clients with Type 1 diabetes (adult and pediatric) or gestational diabetes is considered a strong asset.
A successful candidate in this role would have
· Baccalaureate preparation in Nursing or Food and Nutrition
· Registered Nurse with the College of Nurses of Ontario or, Registered Dietitian with the College of Dietitians of Ontario, in good standing
· Certification as a Diabetes Educator (CDE) preferred
· Experience counselling patients within a diabetes program incorporating various behaviour change models, adult learning principles or techniques for chronic disease management preferred. Experience with insulin therapy; initiation and adjustment, and insulin pumps a definite asset.
· Experience providing diabetes education to adults with prediabetes, Type 2, and/or gestational diabetes; experience with Type 1 diabetes considered a strong asset
· Comprehensive knowledge base in diabetes management is required
· Familiarity of insulin pump therapy, or certification in pump training considered a strong asset.
· Demonstrated ability to work autonomously and as a member of an interdisciplinary team.
· Critical thinking skills
· Comfortable and experienced with individual and group presentations, in person and virtually
· Knowledge of and sensitivity to the impact of social, economic, environmental, and cultural issues in diabetes self-management
· The ability to speak multiple languages would be considered an asset.
· Class G License and vehicle available for use. Travel is an expectation as this position will be involved in outreach activities
In this role you will be expected to complete the following tasks:
· Provide patient‑centred diabetes education to individuals with prediabetes, Type 2, and/or gestational diabetes; education for individuals with Type 1 diabetes as an asset to the program
· Maintain a large and busy caseload.
· Meet 6-7 patients daily for initial referrals and follow up appointments
· Prepare, plan, deliver, and document weekly classes using hybrid options (a mix of in-person and virtual) for up to 25 people per group.
· Provide education related to diabetes and the disease process; medication, diet, foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection.
· Review blood glucose readings for the purpose of medication control and adjustment
· Provide ongoing assessment and recommendations to the treatment team including the patient’s healthcare providers and caregivers for adjustments in the plan of care as required
· Document assessments, interventions and plans including recommendations and goals, progress notes and discharge summaries

· Starting Salary of $33.07-$39.25 Hourly
