Responsibilities:
1. Reviews patient status through methods such as reviewing the patient chart and
other written information, patient observation, interviewing patient and family
members, completion of questionnaires, collateral information and consultation with
peers and other members of the health care team.
2. Discusses, plans and evaluates nursing care and therapies of clients using clinical
judgement and evidence-based practice and working together with physicians,
clinicians, social workers, peer support workers, and other professionals at the
Foundry Centre and in the community.
3. Encourages patients to participate in rehabilitation, work functions, recreational
community and other activities to build self-confidence and to develop life skills and
relationships with others. Evaluates clients adjustments to the community and
general progress. Prepares reports and/or discusses client progress with other
professionals.
4. Performs nursing care to clients such as administering prescribed medications and
treatments and observing, recording and reporting symptoms and changes in
patient conditions. Consults with professional practice or Primary Care leadership on
scope of practice or limitations.
5. Provides counselling, guidance and support to clients and their families and/or
caregivers in health care and treatment including details in home care.
6. Carries out established procedures to prepare essential facilities, equipment and
supplies. May assist physicians in examinations, treatments minor surgery and care
delivery.
7. Provides or assists with emergencies consistent with legislative parameters and
approved organization policy. May perform record keeping and other clerical duties
as required.
8. Trains support staff to carry out individual health care procedures in accordance
with relevant regulations and established internal procedures.
9. May provide in-home health education and registered nursing care
10. Documents information including assessments, plan of care, patient status and
progress and discharge plans in a manual and/or computerized environment and
utilizes all required records management systems for Foundry Sea to Sky.
